Creating a survey

Hi-
I need help in creating a survey. I would like to create a survey similar to the ones where people receive them through e-mail and are able to fill in the bubble corresponding to their answer just by clicking the mouse. What I would like to do is to create a survey from an existing Microsoft Word document and format it so that people can fill in the bubbles by clicking the mouse. Does anyone have advice on how to do this? Thanks for your help. I greatly appreciate it.

I thought it was best to keep it simple. Therefore, I decided just to use Microsoft Word to allow people to fill it out just by typing in the answer next to the question. Once they are finished they can send it back.

Good idea, since there are programs out there that can make surveys and tabluate the results for you, but they cost mega-bucks.

Way to improvise!
